Please fill out the PDF application and submit it to our office. Remember that you must live in Union, Davidson, Stanly, Cabarrus or Rowan County, have an income of less than $65,000 per year and someone in the home must have a intellectual and/or developmental disability.
We screen each application individually during our management team meetings that we hold every other month. We strive to approve each and every application we receive however, due to budget constraints or other factors it is not always possible to approve all applications.
Once your application has been approved, we go to work on coordinating all of the moving parts that will make the project a success. In regards to home modifications, the entire process could take up to six months to fully complete.
